Founding Membership Enrollment 

We're excited to welcome our first 125 Founding Memberships to Direct Primary Care at Integrate Functional Health. A membership may be for an individual, couple, or family, depending on the plan you choose.

As a thank you to those who have supported us over the years, current Stittleburg Restorative Health Care patients will receive priority access to enrollment before memberships open to the public.

​

To reserve your membership, a 50% deposit is required at enrollment. The remaining balance will be charged when your care begins with our anticipated opening in February 2027. Your deposit will be applied toward your initial 6-month membership.

Once all 125 memberships have been reserved, additional enrollments will be placed on a waiting list.

 

New to our practice? We'd love to welcome you! You can still enroll and reserve your membership. New patients will be accepted after priority enrollment in the order memberships are reserved.
